Terms of Service

1. Introduction
This Terms of Service ("Agreement") outlines the terms and conditions under which Penny L. Cohen ("Service Provider") offers speech and language therapy services. By engaging in these services, clients ("You" or "Client") agree to be bound by the terms set forth in this Agreement. Penny L. Cohen is a licensed and experienced speech and language therapist committed to providing high-quality, compassionate care to all clients.
2. Services Provided
Penny L. Cohen offers a range of speech and language therapy services, including but not limited to assessment, diagnosis, and treatment. Services are tailored to meet the unique needs of each client, following evidence-based practices and in accordance with all applicable laws and professional guidelines.
3. Client Responsibilities
Clients are expected to provide accurate and complete information during consultations and assessments. This information is crucial to the effectiveness of the services provided. Clients must adhere to scheduled appointments and notify Penny L. Cohen at least 24 hours in advance if a cancellation or rescheduling is necessary. Failure to do so may result in a cancellation fee.
4. Privacy and Confidentiality
Penny L. Cohen is dedicated to maintaining the privacy and confidentiality of all client information in compliance with the Health Insurance Portability and Accountability Act (HIPAA) and other relevant privacy laws. Any personal information collected will be used solely for the purpose of providing speech and language therapy services and will not be shared with third parties without the explicit consent of the client, except as required by law.
5. Limitation of Liability
While Penny L. Cohen strives to provide the highest standard of care, she cannot guarantee specific outcomes. The therapy process is a collaborative effort, and progress may vary depending on individual circumstances. By engaging in services, clients acknowledge and accept that Penny L. Cohen shall not be held liable for any damages, losses, or injuries arising from the use of her services, except in cases of gross negligence or willful misconduct.
6. Payment Terms
Payment for services is due at the time of service unless otherwise agreed upon. Accepted payment methods will be determined by mutual agreement between Penny and the client, at Penny's ultimate discretion. Invoices will be provided upon request. Failure to remit payment may result in the suspension of services.
7. Termination of Services
Either party may terminate this Agreement at any time with written notice. In the event of termination, the client remains responsible for any outstanding fees for services rendered. Penny L. Cohen reserves the right to terminate services if a client fails to comply with the terms of this Agreement or engages in behavior that is disruptive, inappropriate, or harmful.
8. Amendments
Penny L. Cohen reserves the right to modify or update this Agreement at any time. Clients will be notified of any changes, and continued use of services after such modifications will constitute acceptance of the revised terms.
9. Governing Law
This Agreement shall be governed by and construed in accordance with the laws of the state of California, without regard to its conflict of law principles. Any disputes arising under this Agreement shall be resolved in the appropriate state or federal courts located in San Diego County, California.
